Applying for DDA Enrollment is the first step toward requesting services. It is a process to determine if you meet developmental disability eligibility criteria. It is not the same as requesting services. If you are new to Washington state or DDA, click here for more information about what to expect and why it’s a good idea to apply.
STEP 1: Apply for DDA Enrollment
Select one of the following two ways to request a DDA enrollment packet, also known as an intake packet:

STEP 2: Send Us Your Application Packet
Email or mail your completed documents to the Intake and Eligibility address for your region or drop them off in person at your local DDA office.

If you would like assistance completing the forms, call the number for your regional DDA office (see list above).
After you send us your packet, we will send you an application received form. It includes contact information for status updates and questions.
STEP 3: Receive Your Determination of Eligibility
After we receive the last past piece of requested documentation, our intake and eligibility specialists will make a determination of eligibility within 30 days and send you a notification packet that explains your determination of eligibility and enrollment.
If you are eligible and enrolled:
You have the option to request services right away or wait until you’re ready for that step. If you need more time to decide, you will be assigned a DDA case manager to contact whenever you’re ready to ask for services. You can also visit a DDA office or use our online request form to ask for services at any time. If we do not hear from you, we’ll assume you would like to wait. In that case, your DDA Case Manager will reach out to you at least once a year to talk to you about service options.
If you are not eligible:
You can file appeal. The appeal steps are included in the notification packet.
STEP 4. Request Services
You can ask for services any time after you are enrolled in DDA. To learn more about that process, click here.
Note: You might not need to be a DDA client to receive the service you need.
- For children: DDA offers personal care through Community First Choice for children whether or not they are DDA clients. Even if you do not have a developmental disability, you might be eligible for personal care.

For adults: Adults that need personal care but are not DDA clients can receive this service through Aging and Long-Term Support Administration.

- Washington State Voter Registration for applicants age 18 or older
- In addition, you will need to submit documents to support that you have a developmental disability, as described in Eligible Conditions Specific to Age and Type of Evidence, such as educational records, psychological records and medical records.
