How to enter a photo in the DSHS Photography Contest

Follow these steps for each photo:

1. Take a photo, or find a great one you took in the past.
2. Have each identifiable person in the photo sign a photo release.
3. Fill out an online Photo Submission Form for EACH photo you want to enter.
4. Send the photo in JPEG format (and if possible, scanned photo releases) to this e-mail address: DSHS_Photos@dshs.wa.gov.
   Send each photo in a separate e-mail. Be sure to include the following information:
   • Your name
   • Your contact information (phone number, email)
   • Photo title
   • How you're sending the photo releases (scanned by email or mail hardcopy)
5. If you can't email a scanned copy of the photo release, please send the photo release to this address.

(Be sure to keep a copy of the original photo release).
