Money Follows the Person Tribal Initiative (MFPTI) is a part of the larger Centers for Medicare and Medicaid funded Money Follows the Person Rebalancing Demonstration. The initiative's goals are to:

  • Identify American Indians/Alaska Natives (AI/AN) who are living in institutions and assist them to return to their community of choice.
  • Develop culturally inclusive service systems and providers to support American Indians/Alaska Natives once they returned to their communities.
  • Support tribal contracts to provide long-term services and supports.

In Washington, the Money follows the Person program known as Roads to Community Living has always been an inclusive service providing pathways to home for anyone who was living in a facility. The Initiative focuses on developing inclusive service contracts and engaging potential partners at the state, tribal and county levels for improved and culturally inclusive service delivery of ALTSA services, including tribal contracted support services.

This program is currently available in five states: Washington, Oklahoma, Wisconsin, North Dakota, and Minnesota. Annually, ALTSA, tribes and other partners come together to share successes, best practices and network to strengthen tribally inclusive services and cross-governmental and private industry understanding of the needs of American Indian/Alaska Native elders and individuals with disabilities.

White is north, it is the image of a buffalo photographed in the Theodore Roosevelt National Park.

Red is south, an image of a bird of prey, often depicted in the art, songs and oral histories of Native
Americans including those of the south and southwest.

Yellow is east, an image of a loon and is a clan symbol for the Ojibwe people who have tribes in both
Minnesota and Wisconsin.

Black is west, the image of a salmon which is often found in northwest tribal art.
