Individual and Family Services Waiver

The Individual and Family Services waiver supports individuals with intellectual and developmental disabilities who live with their family to develop and implement supports and services to successfully maintain individuals in their family homes and communities.

The IFS Waiver is a partnership between the Developmental Disabilities Administration (DDA) and families to:

  • Support DDA-eligible individuals living in the family home or who are in a State or Tribal dependency placement with their family.
  • Provide waiver participants/families with a choice of services; and
  • Allow individuals more control over the resources allocated to them.

How to Request IFS Waiver Services

If you have not yet applied to DDA for determination of eligibility please click here to be routed to the DDA eligibility site.  You will need to submit eligibility paperwork before requesting IFS waiver services. If you have applied and been determined eligible for DDA but do not have a Case Resource Manager (CRM), please see the Service Referral and Information Request Page
