In addition to other supports for adults and children & youth, we offer a variety of services and items to help you be more independent at home and in your community. Eligibility depends on your program eligibility and assessed need. Please talk to your DDA case manager about the program and services that work best for you.

Home and Community Based Services Waivers

Please Note: Waiver programs are not an entitlement. This means that even if you are eligible, we may not have enrollment openings.
Community First Choice (All Ages)
Community First Choice services are provided in your own home, an adult family home or an assisted living facility. CFC is available through both DDA and Aging and Long-Term Support Administration.
If you do not qualify for DDA enrollment, you may be still eligible for CFC services, and possibly other services. Adults who do not qualify for DDA enrollment may be eligible for services through ALTSA. Children who do not qualify for DDA enrollment may still be eligible for CFC through DDA’s Specialized Unit.
Personal Care and More:
With CFC, you can get assistance with daily life activities—such as bathing, dressing, eating and taking medication—with the help of personal care services. Personal care is provided by an agency or an Individual Provider you choose.
